Referring doctors can use our standardized form to refer patients for rheumatologic evaluation. Of course, a short narrative description of the patient issues is always very helpful.

Ontario Trillium Drug Program
The Trillium Drug Program is for people who spend approximately 3 to 4% or more of their after-tax household income on prescription-drug costs. To qualify you must:
Not already qualify for Ontario Drug Benefit (e.g. you’re under 65 years old and not enrolled in a program such as Ontario Works) and
Have a valid Ontario health card
Have completed your taxes

Disabled Parking Permit
Accessible parking permits are issued to a person or business and not a vehicle. Permit holders must have been in the vehicle and the permit displayed on the dashboard in order to park in an accessible parking space.
